According to the public relations report of Tehran University of Medical Sciences, Tehran Heart Canter, in the hospital of Tehran Heart Centre, the rare operation of installing an artificial valve inside the previous artificial valve was successfully performed.

This unique work was successfully completed for the first time in the world by Dr. Ali Mohammad Haji Zainali, Cardiology Specialist, with the cooperation of Dr. Kiyomarth Abbasi, Cardiac Surgery Specialist, and Dr. Mohammad Sahib Jam, Echocardiography Specialist. Dr. Zain Ali explained about this action.

Professor, please explain to us about this very important practice.

The operation that was performed is called the operation of the heart valve inside the previous artificial valve or the so-called Valve in Valve Implantation in a non-surgical way, and we know that to install the valve, the chest must be opened and the heart valve replaced.

In the past years, valves have been installed in the world that, like a stent or a spring, we can enter through the foot without surgery and place them on the previous valves. For several years, we have started this work on common valves in aortic, mitral, and even pulmonary and tricuspid positions (the four famous heart valves) and have done it successfully.

But this work, which was done for the first time in the world, was a special patient who was 25 years old and had previously been operated on once in 6 months due to aortic valve stenosis and twice after that due to the recurrence of stenosis of the same valve.It was no longer possible to open the chest and operate on the patient. In the last operation, which was done about 7 years ago, from the tip of the heart, which we call the apex, to the aorta, a duct and a valve were placed by Dr. Abbasi, a scientist surgeon who was also a member of this team and this work, and this The title was the last way to treat this dear patient.

That is, his own aortic valve that was damaged (artificial valve) was placed into the aorta through the tip of the heart and a valve was placed to save this young man 7 years ago. Unfortunately, that valve was also damaged, that is, a valve that was in an unusual position and was the last resort 7 years ago. In the past few months, this valve had failed again and there was no other option or choice for treatment.

By checking in the articles in the world and even talking on the phone with colleagues who are abroad, we asked what is your suggestion, we saw that there is no way and in fact we have to do something risky and enter that channel on foot and that The valve that was placed in a very abnormal place at the tip of the heart, put this valve on it, which is called Valve in Valve in the apico-aortic condo where this risk was taken from this abnormal route.

This work was done as a team, with the presence of Dr. Abbasi, a heart surgeon, and Dr. Sahib Jam, an echo specialist, because it is the first case of using this type of Edwards valve in the world in this duct.

What is the frequency of such patients?

The frequency of valve-on-valve operation within the heart itself is very high. We do this for patients who were operated once and put a valve and that valve broke again so that their chest does not open again. There are many patients in the country who have an artificial heart valve and for them it has the advantage that If their heart valve is damaged, a second open operation is no longer necessary and they can put this valve on that working valve, but in this particular patient, it has a universal message because there are such patients in the world who took risks and opened the operation. Edwards Company and the centers in Europe and America said that if you do this new work and if you succeed, it will definitely be a way and a light for us to be able to follow this path from now on.

Usually, a procedure that is successfully performed on a human for the first time in the world is referred to by others. The biggest advantage of this work is that now the name of the heart center, our country and our university is mentioned as the first in the world, and one year from now, this work will certainly be done in many places in America and Europe with reference to the patient of this centre.
